Acting like so-called Prime Minister, the Miko of the Great Hakurei Empire take ultimate responsibility for the true Kami, Hakurei Reimu, the Queen, Remilia Scarlet and the Senate of the nation. Commonly believed as the incarnation of Hakurei Reimu though, most of the information about this so-called Doctor Reimu remains classified. Yet the fact that he/she acts as the regent missionary of Kami's will is solidly confirmed.

Apart from such position, he/she also acts as the Miko, or so-called Pope by common sense of the one and only religion towards Hakurei Reimu with the right to perfrom, reform and abolish every kind of rituals within the region.

Rumours argue about the existance of a special knighthood called Round Table of Miko's Divinity and doubt whether Doctor Reimu is the executive commander for all operations.
